The elegant article by Lee et al1 demonstrates the efficacy of optimizing liver volume drainage with bilateral stent placement for hilar cholangiocarcinoma as opposed to unilateral stent placement. The technical success rates of over 95% in the bilateral stent placement group are testament to a highly experienced and skilled team. Despite the difference in stent patency and reintervention rate favoring the bilateral stenting approach, there are some issues with the article and the topic in general that need clarification.
